This form outlines the essential details, procedures, and potential risks associated with tattooing and piercing. By completing this form, clients confirm their understanding of the tattoo and/or piercing process, acknowledge potential risks, and provide their consent to proceed with the tattoo/piercing work at Sting Studio.

I hereby declare that I am of legal age (and will provided valid identification) and understand that Sting Studio Inc.'s free touch up policy don't apply to hands, face or feet. *

I acknowledge I will take great care of my tattoo or piercing during the healing process. I understand the artist may decide at their discretion to deny a free touch up if there is tell-tale signs of lack of effort to follow healing procedures. *

I consent to having the procedure captured on film for the purpose of creating professional tattoo reels of the artists at work and the results of their work for Sting Studio Inc. marketing only. Anonymity will be respected if asked. *

I release all individuals at Sting Studio from any liability for personal injury or damages, whether direct or consequential, resulting from the tattoo or piercing procedure. *
